 There are medications to be arranged, assisted devices to be purchased and installed, follow-up appointments to be booked, and at-home recovery instructions to be managed; not to mention transportation and other important activities. 

We coordinate everything with your hospital Discharge Team. We will take the time to understand your individual needs and design a plan that will take the stress out of moving home so you can concentrate on your recovery! 

Rehab and Recreational Support Worker will assist with carryover of physiotherapy  goals, practice functional tasks and serve as a positive companion to attend meaningful and engaging activities, including community integration.
